Important Initial Consultation Guidelines
All required health documents, including lab results and medical history, must be submitted one week before your initial appointment to ensure a thorough and effective consultation. This allows Keith McCormick to review your case in detail and provide informed guidance. Please note that these consultations are for educational purposes only and do not replace medical advice from your healthcare provider.
It’s crucial to follow all guidelines carefully to prevent delays or issues with your consultation.

To schedule your consultation with Keith McCormick, please book your appointment online.
- Initial Consultation (45 min to 1 hour): $395 (in person or by phone)
- Follow-up Appointments (15 to 30 minutes): $110 to $175
Information Submission Guidelines
To ensure an effective consultation, please follow these instructions carefully. All required documents must be mailed and received one week prior to your appointment. Email attachments will not be accepted. Please organize your materials as follows:
- Health Summary: A brief, one-page document listing medications, diagnoses, surgeries, etc.
- Bone Mineral Density (DXA) Test Results: Include the full 3-4 page DXA printout, not just the radiologist’s summary. Order them from the most recent to the oldest.
- Lab Results and X-ray Reports: Include test results from the past two years.
All documents should be 8.5 x 11 inches, organized without staples, dividers, or notebooks. Be sure to mail all information in one packet. No more than 60 pages in total, and please avoid unnecessary information (e.g., test orders or repeat data).
Preparing for Your Consultation
Once we receive your information, we will contact you to confirm your consultation details. During the session, Keith McCormick may recommend additional lab tests based on your individual case. Lab tests may require fasting or specific times for accuracy.
